Acceptable Proof Of Age Guidelines

Only the following items will be accepted as proof of age:

1.	U.S. Birth Certificate (State or County)
2.	Notarized and translated foreign Birth Certificate
3.	U.S. Passport
4.	Notarized and translated foreign Passport
5.	U.S. Resident Alien Card
6.	Identification card issued by the DMV
7.	Medical insurance card
8.	Computer generated report card
9.	Embossed Baptismal Certificate
10.	Notarized and translated Embossed Baptismal Certificate

NOTE: Letters from schools and hospital birth certificates will not be accepted.
